Please know your insurance coverage and benefits as we will gladly bill your insurance carrier for services provided, but will hold you responsible for services not covered by your plan.Co-payments are due prior to being seen for your visit.We accept cash, check and credit/debit cards as payment, but any returned checks will be subject to a fee. We ask that you give at least 24 hours notice if you cannot make your scheduled appointment.We hold the right to bill you personally for missed appointments that were not cancelled at least 24 hours in advance.We also hold the right to refuse scheduling of any further appointments if you have 3 no-show appointments, or an outstanding balance.
